A fatal software flaw in older Samsung phones that blocks users from contacting triple zero emergency services may have caused a second death. TPG Telecom’s chief executive revealed to a federal parliament Senate committee that a Blue Mountains resident could have died in September after failing to connect to emergency services via 4G, with contact only established through an alternative method five minutes later.
